Before we begin the training process, we need a few documents from you that include a copy of your:

• Driver’s License

• Professional Liability Insurance (Declaration Pages only)- Each provider is required to have their own liability insurance with the following limits: 1Mil/3Mil

• Social Security Card

• Highest Degree Earned

• Professional License

• Resume

**Associate Licensed Providers (LCMHCA, LCSWA, LMFTA) must include a copy of their supervision contract**
