Step 1- Apply for an individual (type 1) NPI

A National Provider Identifier (NPI) is like a professional social security number. You need an Identity & Access Management System (I&A) User ID and Password to create and manage NPIs. Use the button below to create your I&A User ID. If you already have a NPI, please move to Step 2!

Step 2- Apply for CAQH Number

The Council for Affordable Quality Healthcare (CAQH) was created to “streamline” the insurance credentialing process by operating as a database for insurance companies to “pull” credentialing information from when someone is applying to be a network provider. This application can take several hours to complete and will need to be in “approved” status to move forward with credentialing. You will need the following practice information when adding our group practice to your CAQH profile under Practice Information:

Step 3- Complete our Credentialing Sheet

Our credentialing sheet collects the data necessary to apply for insurances on your behalf. You will need a copy of your highest degree, liability insurance declaration page(s) & a copy of your professional license (supervision contract needed for those who are not fully licensed).
